Investigating Ethical Attitudes and Decisions in a Simulated Cardiac Care Unit:"We have tried to circumvent this problem by using a micro-world computer simulation of a cardiac care unit to study treatment decisions in "life or death" situations. The task facing the physician in this simulation is to be in charge of a 6-bed cardiac care unit for a period of 14 "days" controlling admissions, discharges, and treatment of all patients. The simulation dynamically generates the disease states of the patients in the unit including physical findings, laboratory findings, ECG, X-rays, eccocardiography etc. and their responses to treatment. The simulation also presents new patients from the emergency room, thereby forcing the physician to decide whether the new patient should be admitted, and whether one of the present patients should be discharged if no bed is free."

Internal Medicine:Cardiac Care Unit:"Students will function as sub-interns while on a four-week rotation. Their primary responsibility will be to admit patients to the Cardiology Service and manage their care under the direct supervision of a senior medical resident, cardiology fellow and attending physician. During this time period, students will be expected to work up and present assigned patients on daily rounds with the house staff and faculty. They will also be expected to stay in-house with the senior resident and intern every fourth night, as arranged at the start of the elective. In addition, each student should acquire experience in ECG interpretation as well as observing the techniques of performing and interpreting echocardiograms, Holter monitors, stress tests and the placement of central venous and Swan-Ganz catheters. At least one cardiac catheterization, coronary angioplasty and electrophysiology study should also be observed."
